The $50 Medicare number needs the most caution

The Medicare GLP-1 Bridge is the number most likely to get repeated without its guardrails. CMS announced a $50 monthly access point for GLP-1 medications for eligible Medicare beneficiaries, with the program beginning July 1, 2026. Cash-pay routes are not interchangeable discounts

LillyDirect, NovoCare, Sam’s Club oral Wegovy, and a standard pharmacy-counter cash price should not be lumped together as if they are four coupons for the same checkout lane. They are different routes with different eligibility gates, product forms, doses, fulfillment rules, and refill logistics.

The LillyDirect-Walmart setup is the cleanest example of Walmart being part of the pickup path. Sam’s Club’s oral Wegovy announcement is a different animal. The company said members could access the first FDA-approved oral GLP-1 option, oral Wegovy 1.5 mg, at $149 per month self-pay. That is a very specific route: Sam’s Club, oral Wegovy, 1.5 mg, self-pay. It should not be stretched into “Wegovy costs $149 at Walmart” or “all GLP-1s are now $149.” [3]

NovoCare adds another separate path. Novo Nordisk’s Wegovy payment page lists a self-pay offer that starts at $199 and then moves to $349 per month. The same page also describes an insurance-and-savings-card route as low as $25 per fill for people who meet the terms. Those numbers can be excellent if you qualify. They are also eligibility-sensitive, which means they are exactly the sort of numbers that need to be checked against your insurance type, savings-card rules, and refill channel before you assume they apply.
